The nursing care indicators cover those areas which are the highest concerns in terms of risk:
- Patient observations
- Pain management
- Falls assessment
- Tissue viability
- Nutritional assessment
- Continence assessment
- Medication administration
- Infection prevention and control
- Diabetes monitoring and treatment
- Discharge checklist
- Quality assurance
These indicators are evidence based and follow advice from bodies such as the NPSA, NICE and the Royal College of Nursing. Unannounced audits by senior nurses audit the records of 10 patients every month. We use a ‘Back to the Floor’ model and real time data collection and reporting which is triangulated with existing data to ensure that we have an accurate picture of processes that are in place.
These Metrics measure our standards of record keeping for the core activities that we undertake for our patients. They demonstrate our professionalism and give an accurate and contemporaneous record of our patients care. We can show real time quantitative data – 450 patients are audited every month (5974 records during 2010).
This initiative has created a significant improvement to nurses’ ability to collect and use data to enhance the quality and safety of care. Real time quantitative data means nurses can benchmark their performance. The assurance framework provides a systematic improvement programme. We provide assurance to the Trust Board. Wards have been successful in raising their performance when measured against the Metrics, signifying an improvement for safety and the patient experience. Good practice has been shared and celebrated. Senior Nurses on the wards every month has given opportunities for networking, trouble-shooting, support and teaching. A competitive element has developed and targeted support has been given to those areas which have difficulty in achieving a satisfactory score.
